I, on behalf of myself and my family members, give permission to Thornhill Park Tennis Club (the “Club”) to capture/make images, including photographs and/or videos, of the named participant, for Club activities and for promoting the Club. I also give permission for images of the named participant to be copied, used, exhibited, displayed, broadcasted, distributed and posted, including derivative works and recordings in any format or media now or in the future. I understand that images of the named participant will become the property of the Club, and be kept for use by the Club. I understand that I have the right to revoke consent, at any time, to future use of images of the named participant by contacting the Club and providing a way of identifying the photo, video or recording concerned.
